- Can anyone submit an Electronic Service Request (ESR)?
No, anyone who submits an ESR must have a Signature Authorization Card on file with DHR.
- How many email notifications will be sent after I submit an Electronic Service Request (ESR)?
You will receive two auto-generated email notifications. The first email is confirmation that your ESR submission was received and the second email is confirmation that your ESR has been completed.
- How do I attach additional documents to my ESR?
Additional documents can be attached after submitting your ESR. A link in the confirmation of submission email will allow you to add any additional documents.

Can I submit my ESR with multiple email addresses?
Yes, you can add multiple email addresses separated by commas or semicolons.
- How can I tell who is currently processing myESR ?
Your personal dashboard contains key information, including who it is currently processing your ESR. A link to your dashboard is included in your confirmation of submission email.
- What are the required fields on the RTF, AP, and CH Review ESR?
The required fields vary based on the type of ESRs. If you forget to fill in a required field, you will be prompted to fill in that field prior to submitting your ESR.
- Can I use Adobe to submit an ESR?
No, Adobe Professional is currently not supported. However, SmartPDF is testing Adobe Products and we will update our website when testing has been completed.
Please be sure to use Nuance Professional for all your ESR’s.
Request to Fill/Modify Electronic Service Request (RTF ESR)
- Does the department need to attach an ECQ, JAQ-Regular OR JAQ-Management Form and Organizational Chart for each request?
DHR no longer requires departments to submit an Org Chart with every RTF ESR, however, the appropriate classification form must be attached to the submission.
- If a Special Condition is not listed on the PeopleSoft list, can I still make a SC request on my RTF ESR?
When a department is requesting to use a Special Condition, a Special Condition Request form is required to be attached to the Request to Fill/Modify ESR regardless of whether a code already exists or not. If no pre-existing special condition code exists in PeopleSoft, there is a section on the Special Condition Request form for requesting the creation of a new special condition code.

Why is fingerprinting now done pre-employment?
The review of a candidate’s conviction history information has always been a part of the City’s pre-employment verification process. In the past, departments have relied on a process of self-disclosure of conviction history from the candidate to make the requisite nexus determinations, pursuant to Civil Service Commission policy, prior to the extension of an offer of employment. Fingerprints were obtained post-employment from the individual and used to verify the self-disclosure. In preparation for the elimination of the self-disclosure process in early 2013, the pre-employment fingerprinting requirement was implemented. Ultimately, the only source of a candidate’s conviction history information will be from the reports obtained through a fingerprint check by the California Department of Justice and the Federal Bureau of Investigations.
